Render operations against state, applying each to a cloned state so later ops see earlier ones (needed for SQLite rebuilds).

Errors

MigrationError::UnsupportedBackend for anything other than PostgreSQL, SQLite or MySQL, or MigrationError::State when an op cannot be rendered.
