Deterministic schema diff: diff compares two ProjectStates.

Renames are emitted only when listed in RenameHints. Without a hint a renamed field is RemoveField + AddField (the column is dropped and recreated; data is not preserved) and a renamed model is DeleteModel + CreateModel. Those unhinted pairs are refused when the dropped and added table or column have the same shape.
